Support clients with personal care, mobility, nutrition, medication prompting, and daily living activities. Promote clients' independence, dignity, and wellbeing in their own homes. Monitor, record, and report changes in clients' physical, emotional, and social wellbeing. Maintain accurate care records and documentation. Communicate effectively with care managers, nurses, families, and other healthcare professionals. Ensure a safe environment by following infection prevention, PPE, and moving & handling procedures. Support clients with complex care needs and escalate concerns when appropriate. Mentor and support new carers through shadowing, induction, and sharing best practice. Assist with competency observations, feedback, and identifying training needs. Promote positive communication between carers and the office team.
